Living with Limb Length Discrepancy: Non-Surgical vs. Surgical Options

Not every limb length discrepancy needs surgery. The right approach depends largely on the size of the difference, the patient’s age, and how much it’s affecting daily life.

For small discrepancies — often those under about 2 centimeters — non-surgical options are usually tried first. A shoe lift or custom orthotic insert can even out pelvic and gait mechanics without any procedure at all. Physiotherapy can also help address compensatory posture habits and strengthen supporting muscles, particularly around the hips and core, which often take on extra strain when one leg is shorter.

When the discrepancy is more significant, or when a lift becomes impractical (imagine trying to build a two-inch lift into a shoe), surgical options come into play. In growing children, a technique called guided growth can slow the growth plate on the longer leg, allowing the shorter leg to catch up naturally over time. This is a much smaller procedure than full lengthening and works best when there’s enough remaining growth to make use of.

In adults, or in children with larger discrepancies, surgical limb lengthening becomes the primary option. This uses the distraction osteogenesis process — the bone is cut, an internal lengthening nail is placed, and the bone is gradually stretched over weeks until the discrepancy is resolved.

There’s also the option of shortening the longer leg in select cases, though shortening is actually a lot more demanding on the patient’s muscles and recovery compared to lengthening. Lengthening the shorter leg is usually preferred instead, since shortening means sacrificing healthy bone length and strength on the unaffected side.

The right path is rarely obvious from the outside — a 1.5 centimeter difference in a growing eight-year-old is managed completely differently than the same difference in a 40-year-old with hip pain. That’s why a full evaluation, including imaging and a discussion of long-term goals with someone specialized in this field, should come before deciding on any treatment.
